Maximus is hiring Community Health Workers (Sr CSR – Outreach) to work with families eligible for Medicaid to educate, assist and enroll in Medicaid Managed Care and learn about preventive health care in Texas Health Steps. Jobs are available for people familiar with these communities in Texas and are Work from Home in the community you serve. Day travel for in person Home Visits and Community Event attendance are required.

Job detail includes attending enrollment and health events, conducting home visits and presentations to promote and educate STAR, STAR+PLUS or STAR Kids and Texas Health Steps. Outbound phone calls conducted prior to home visits. Mileage reimbursed. Mobile phone issued.

These are full-time positions with a flexible schedule to attend events and conduct recipient outreach. Positions are work-from-home with regional offices based out of Abilene, Arlington, Athens, San Antonio, Houston.

Aide, Community Service, WIC - Public Health

Summary: Assist with infant mortality prevention and WIC outreach in the community. 

Minimum Requirements: Certification as Community Health Worker required or, in lieu of the certification, proof of at least 1000 cumulative hours of community health work services within the most recent three (3) years. One (1) year of experience as Community Health Worker. Two (2) years of WIC experience. Organizational and customer service skills required. Must have interpersonal skills to relate well to customers and other agency personnel. Current and valid driver license is required.

Community Health Worker - Equality Health, Dallas

The Community Health Worker (CHW) is primarily responsible for providing supportive care and assistance to a diverse patient population to identify barriers to self-care and healing, decision making and communication in navigating the healthcare system. Community Health Worker 2

Job Summary: The Community Health Worker 2 is responsible for providing cultural mediation between the health care system, social services and the community. This position assists the community in gaining access to needed services and builds individual, community and system capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, patient navigation, follow-up, community health education and information, informal counseling, social support, advocacy and participation in clinical research.

Clinical Outcomes Associate  - Cook Children's, Fort Worth

Responsible for developing and maintaining relationships with CCHP members and providers through outgoing telephonic outreach. Promotes positive customer relations through prompt, courteous, and professional service in this fast paced, high volume, customer centered environment. Additionally, will be responsible for answering incoming calls and responding to inquiries relating to providers, benefits, member accounts, other health insurance and health plan policies. Work closely all frontline staff, providers, and the member population to improve member engagement, leading to improved health outcomes. This position is responsible for applying appropriate engagement tactics to close care gaps, promote wellness screenings, and facilitate the completion of Health Risk Assessments (HRAs). Because of the diverse nature of our Member population and nature of assigned duties, must be bilingual in English and Spanish.

Community Health Worker - Anthem, Fort Worth

The Community Health Worker is responsible for serving as a liaison/connector between the patient, care team, and the community. Helps to bridge conversations with patients and remove barriers that prevent them from accessing health and social services. Conducts telephonic and/or face-to-face outreach to panel of patients for appointment scheduling, needs assessment, and care gap closure.

Minimum Requirements:
- Requires a High School diploma or GED.
- Experience utilizing electronic medical record.
- A minimum of 1 year experience in a healthcare, community-based, or social work environment; or any combination of education and experience which would provide an equivalent background.
- Travel may be required (Fort Worth, TX area).
